2008 Family Choice Awards (FCA)

Parenting Resource Entry Form

Entry Criteria / Instructions

  • Products should be intended for parent's use only
  • Products that have been re-released with updated features will be considered as new products
  • Complete and Sign entry form per product
  • 4 samples of each item is required with total payment
  • High resolution image (300 dpi or better) of each product in a JPEG, TIFF, PDF, or EPS file should be submitted to FCA@FamilyMagazineGroup.com

Judgment / Winner

Each product is reviewed and judged according to the following criteria's:
  • Quality and Appearance
  • Practicality
  • Educational Value
  • Level of Interest
  • Price
Winner will have met the highest standard of all criteria's with an excellent overall presentation.

Winner will be notified in advance and will receive 1 year print coverage & 1 year of coverage online. Winner will also receive a Family Choice Award plaque for display. 2009 Family Choice Award seals are available online to winners only.Ordering information will be emailed to winners.

Payment

Payment must be paid in full for each entry in U.S. funds at time of submission.

Total number of entry must be indicated on payment form.

3-4 Submissions: $75 each
5 or more submissions: $50 each
